Referral Response Process
Submitting a referral does not automatically mean acceptance. It does mean the case enters a structured review that ends with a clear answer.
Submit basic details and supporting clinical documents securely. Liberty accepts referrals from hospitals, rehabilitation centers, physicians, case managers, social workers, MCOs, and community organizations.
A nurse reviews the patient's needs, location, timing, payer, orders, and staffing requirements before any decision is made.
Liberty determines whether the requested service can be provided and what authorizations may be necessary.
When appropriate, Liberty coordinates an assessment and an individualized plan of care with the physician.
Qualified nurses are matched according to the patient's clinical and scheduling needs.

Dr. Rollins-Munroe brings more than 21 years of nursing experience across intermediate care units, emergency services, population and community health, nursing management, and nursing education. Her career began as an LPN in 2004 and advanced through BSN, MSN, and PhD.
As Liberty's Director of Nursing, she leads clinical oversight, care planning standards, supervision protocols, and quality assurance across all cases, so families and referral sources can expect one standard of care no matter the assignment.

Liberty Advanced Nursing was founded by Vashon Hargrove, whose work with older adults and families extends across in-home care, housing, accessibility, home safety, and senior relocation. In addition to leading Liberty, he is a Certified Aging-in-Place Specialist, Senior Home Safety Specialist, Maryland-licensed home improvement contractor, and longtime licensed real-estate professional.
That background gives Liberty a broader understanding of the environment in which care must actually succeed: the patient's home, accessibility, family support, safety risks, and long-term living plans.

When the Home Itself Needs Attention
Unsafe bathrooms, difficult entrances, fall hazards, poor accessibility, or spaces that no longer support the patient's needs can undermine even the best clinical plan.
